There is no prescribed form to sign for health and welfare decision-making in Northern Ireland today. Do not use a form from England, Wales, or Scotland — it has no legal effect here.
If capacity is disputed, the appointment is complex, or property crosses jurisdictions, get advice from a qualified lawyer or notary where appropriate.
After it is properly completed
Not applicable. Discuss your wishes with family and health-care providers, and get legal advice on what informal or common-law options exist in the meantime.
