Before you sign
This LPA only takes effect once you lack capacity to make the decision yourself, and only applies if you specifically give the attorney authority over life-sustaining treatment.
If capacity is disputed, the appointment is complex, or property crosses jurisdictions, get advice from a qualified lawyer or notary where appropriate.
After it is properly completed
Give copies to your attorney and share the registration details with your GP or care providers so it can be acted on when needed.
You can then record its location and the responsible contacts in BeforeHand. Uploading a file does not make it legally valid or replace delivery, registration, witnessing, or notarization required elsewhere.
